    We do not require a witness signature on procedural consents. We removed it from policy last year. The witness was only for the signature, not what was discussed.     Hello Melissa, We are accredited by DNV also, and the provider can serve as the witness to the patient's signature. We require a second witness in the case of telephone consent. I hope this helps, I have attached our policy please let me know if you have any questions. Thanks Nellie
