FMEA or Other Risk Management Tools

I am needing risk management FMEA or other assessment tools to complete a project and unfortunately when I moved to this job I did not take all of the materials that I should have with me and those documents have since then been disposed of.  Can anyone help me with the basic fmea tools (scope, detection scale. Spreadsheet)?

You might consider using the Prospective Hazard Analysis Toolkit, which can be found at the end of this (very long) report:

Full disclosure: It was developed by some colleagues of mine at the University of Cambridge. But I've used it, myself, and found it helpful.
