Controlled Substance agreements

I have a question for anyone who uses controlled substance agreements at your organization. Our agreement allows for termination of a patient from our practice for breach of contract however we have a couple of patients who we don't want to terminate because they have serious medical conditions that need monitoring. We have communicated verbally to the patient that we will continue to provide services but we will not prescribe controlled substances. Has anyone else dealt with this? If so, do you have a sample letter to send to the patient that you wouldn't mind sharing?

We use a pain management agreement when narcotic pain management is anticipated to be part of the treatment plan for longer than an acute medical issue, and the agreement includes the potential for termination of narcotic medications. However, unless there is a concurrent disruptive behavior, the patient is not automatically terminated from other treatments. I would be happy to send you samples of our pain management agreement.
