I have been reviewing medical records for almost thirty years until just recently by reading page by page. I think I had become pretty proficient in reading between the lines and spotting patient injuries caused by medical care etc. Recently however my hospital has implemented a EMR or EHR or whatever the correct abbreviation might be. I can no longer read the record page by page because of the length and also because there is little to no flow of the information as it comes out of the system. Pages upon pages of vital signs, separated from narrative notes (of which there aren't any), and assessments that cover a dozen pages each.
I'd like to reach out to the EHR industry for help. There needs to be an electronic solution available to risk managers and others to retrospectively review charts. I would suggest reports based on "triggers' or even a report that gives all information chronologically for specified time frames. Doesn't seem like rocket science to me, just needs some attention. Maybe some younger, computer sauvy risk managers have other ideas, be happy to hear about them. BQ
