I'm wondering what role risk managers have had in the current trend to document every little piece of inforamtion over and over again until the record becomes indecipherable. Did we win the documentation war? Or did we create a worthless medical record? Admittedly I don't take care of patients and perhaps the EMR is great for that purpose, though that's not what I hear from staff. If it is correct, then great, that is always our objective. But from a clinical reviewer's point of view, trying to figure out what happened to the patient, the EMR is almost impossible to use. Perhaps CMS can dvelop a few thousand more codes to help us!

Is it time to start pushing back on the numerous and ever increasing requirements for documentation?
